Eligibility Requirements:
Have a specified Federal Annual Adjusted Gross Income
Are HIV positive and have a valid prescription
Are not covered by/eligible for Medi-Cal or other third party payor (ADAP share of cost/co-pay coverage may be possible)
Are a resident of California
Are 18 years of age or older
What to bring?
California Driver's License or picture ID
Social Security card
HIV diagnosis status, including your last T-cell viral load and the date of your original diagnosis
Name, address and phone number of the prescribing physician
Proof of income, including any one of the following: last year's tax return, two paycheck stubs, a bank statement, an award letter from public assistance agency, a notarized income affidavit provided by eligibility workers
Proof of residence in the state of California
Proof of any public or private insurance
